Two studies conducted in 2018 and 2022 by Save the Children, which has had a stable presence in the occupied Palestinian territories since 1973, evidenced this. The 2022 survey showed that the psychosocial well-being of children and their caregivers had further declined compared to 2018. In those four years, the percentage of those feeling sad or depressed increased from 62% to 77%; anxious from 50% to 84%; and nervous from 55% to 80%. In 2022, children reported feeling more insecure – not only when away from their parents (in 9 out of 10 cases) but also at home (44% felt unsafe compared to 29% in 2018). A variety of symptoms manifested their distress: 9 out of 10 showed aggressive behaviour or irritability, and more than half had difficulty speaking or communicating in general.

Children’s Feelings in Gaza (in percentage)

Source: Save the Children.

Children’s Sense of Insecurity (in percentage)

Source: Save the Children.

Children as Primary Victims of the Conflict

As of the time of writing, we are in the seventh month of the conflict: in 220 days of Israeli government attacks in the Gaza Strip, there have been 35,303 casualties, but only 24,686 have been identified, and the numbers change daily. Due to identification difficulties, estimates of the number of women and children among the victims are not always consistent. The Palestinian Ministry of Health estimates that more than 14,000 minors have died during the conflict, while the UN Office for the Coordination of Humanitarian Affairs (OCHA) currently provides equally alarming data on identified victims: over half are women (20%) and minors (32% or 7,797 children). In addition to the staggering number of victims, over 78,000 people have been injured. Seventy-five percent of the population (1.7 million people) are displaced, including unaccompanied minors living in shelters where the average space available is one square meter, often lacking ventilation systems (41% of shelters) and experiencing leakage issues (57% of cases).

Israeli military activity has caused unprecedented destruction of homes, services, and civilian infrastructure, closely monitored by UN human rights experts, the International Court of Justice, and the International Criminal Court. Children are being killed at a devastating rate, and entire families are being wiped out.

The Health Condition of Gaza’s Children

Limited access to clean water and the overcrowded living conditions of the population, combined with poor hygiene and a reduced variety of food, have meant that the health of the entire population in the Gaza Strip has severely deteriorated. Between October 16, 2023 and May 1, 2024 there have been over 761,000 cases of acute respiratory infection, over 415,000 cases of dysentery (with one in four cases involving children under five), approximately 91,000 cases of scabies and lice, over 8,000 cases of chickenpox, and 61,000 cases of acute jaundice, according to World Health Organization data.

The poor sanitary conditions particularly affect children and pregnant women. A February 2024 survey by the Global Nutrition Cluster showed that 90% of children aged 6 to 23 months and pregnant or lactating women suffer from severe nutritional deficiencies because the available food has very low nutritional value. Furthermore, at least 90% of children under five have one or more infectious diseases, and 70% have suffered from dysentery. An increasing number of children depend on powdered milk for survival, but this requires safe and clean water, a scarce resource in the Strip where 81% of families do not have access to it, and the average availability is one litre per person per day (while the minimum standard is 15 litres per person per day). The scarcity of clean drinking water exposes the youngest children to increased risk of infection and malnutrition.

In May 2024 OCHA estimated that over 50,000 children suffer from acute malnutrition, with approximately 31% of children under two in northern Gaza and 10% of those in Rafah affected. According to the Integrated Food Security Phase Classification (IPC), famine is imminent as the Israeli government continues to obstruct the entry of aid to Gaza, restricting access to adequate food, water, sanitation, and health services for children and families in dire need. Any use of starvation as a method of warfare is strictly prohibited under international law, and denying access to assistance is a severe violation of children’s rights.

The shortage of food, water, fuel, and medical supplies, along with the dismantling of the healthcare system, seriously jeopardizes the provision of health services, access to medical care, and the delivery of life-saving medicines and equipment. Continuous attacks on healthcare facilities and medical personnel have severely strained Gaza’s health system. As of May 7, there have been 447 attacks on healthcare facilities: 155 buildings, including 19 mobile clinics, and 304 ambulances have been affected, resulting in the deaths of 493 healthcare workers. The impact on the healthcare infrastructure has been severe: while all nine existing field hospitals are fully operational, the situation is more dire for hospitals and primary care centres. Only one in three hospitals is partially functional at the moment, while about 70% of primary care centres are not operational.

Functionality of Health Facilities in Gaza by Type

Note: Data as of May 8, 2024. Source: World Health Organization.

Currently, Save the Children’s emergency unit is working with partner organizations to provide pediatric medical support at the Al-Mawasi field hospital. The unit’s doctors treat war injuries daily, support severely ill children, mothers, and newborns, and train other doctors and nurses on dehydration and burn cases. The war makes it very difficult for all humanitarian organizations to continue operating meaningfully and consistently within the Gaza Strip. Despite border crossing restrictions for goods, Save the Children and its field partners continue negotiating the entry of new trucks to distribute essential supplies, bringing food, water, medicines, hygiene kits, and supplies for displaced families.

Missing Palestinian Children

Thousands of Palestinian minors are missing: detained and forcibly transferred to Israel, their bodies unrecoverable under the rubble, damaged beyond recognition by explosive weapons, buried in unmarked or mass graves, or lost in the chaos of war. Humanitarian organizations demand that the deaths of minors be officially recorded, their families informed, and accountability established.

According to Save the Children’s 2020 and 2023 reports “Defenceless” and “Injustice,” the reports of detention and mistreatment of Palestinian children in Gaza by Israeli forces are extremely concerning. Palestinian children have been experiencing a protection crisis for years. As reported by B’Tselem, the Israeli Information Center for Human Rights in the Occupied Territories, children from the West Bank, Gaza, and East Jerusalem are arbitrarily detained in Israeli military prisons, unable to enjoy the right to a fair trial or basic guarantees. Israeli authorities repeatedly force Palestinian families to relocate, pushing them into increasingly smaller areas without any security or return guarantees, with inadequate infrastructure and services to sustain life. It is estimated that it will take 14 years to clear Gaza of unexploded ordnance and that each square metre is covered by over 200 kilogrammes of rubble. Children in Gaza no longer have a safe place to go, facing risks from unexploded ordnance, direct bombings, or losing their family members.

Bio

Silvia Gison is an International Humanitarian Advocacy and Policy Officer for Save the Children Italy.
